Community Health Connections (CHC) is a multi-site, non-profit health care center offering walk in urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral and mental health services for children and adults, substance use disorder treatment, specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, high quality health care regardless of income or health insurance status. CHC serves all – insured, underinsured and uninsured, low income, publicly housed and homeless populations.

CHC was founded in 2002 and has decades of experience as a Federally Qualified Health Center (FQHC). CHC has five locations within Fitchburg, Gardner and Leominster and serves 36 communities in North Central Massachusetts. All CHC health centers are located along the Montachusett Regional Transit Authority (MART) bus line.
