Community Health Connections (CHC) is a multi-site, non-profit community health care center offering walk in urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral and mental health services for children and adults, substance use disorder treatment, specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, high quality health care regardless of income or health insurance status. CHC serves all – insured, underinsured and uninsured, low income, publicly housed and homeless populations.

Community Health Connections receives HHS funding and has federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
