Community Health Connections is all about community.

We work best when working together with community organizations, supporting each other for outreach, education, care, and support for just about every need an individual could have. Our partners range in specialty and size, yet are all incredible at what they do and how they serve the community.

Anchor Collaborative – CHNA 9

CHC has been and continues to participate in a variety of Community Health Network of North Central Massachusetts’ (CHNA9) initiatives, including serving as a partner in their Anchor Collaborative. Through this participation, CHC is commiting to investing in and leading by example within the North Central Massachusetts community, gathering insights, hearing the unheard, advocating on their behalf, and helping provide a deeper understanding of the populations that make up our communities.

Through identifying needs expressed by our area’s diverse stakeholders and implementing collaborative solutions as an organization and community leader, we are and aim to continue making our community stronger.

 
 

Growing Places

Community Health Connections partners with Growing Places to have CHC’s Nichols Road location serve as a weekly stop to purchase fresh produce. Everyone is welcome to shop anytime.

Fitchburg Art Museum

Community Health Connections partners with the Fitchburg Art Museum to showcase local art from the museum at our Fitchburg, Gardner, and Leominster locations.

Fitchburg State University

Community Health Connections partners with Fitchburg State University to provide healthcare services to all students.

Mount Wachusett Community College Dental Assistant & Hygiene Programs

Community Health Connections partners with Mount Wachusett Community College to educate Dental Assistants and Dental Hygienists. The programs are located at Fitchburg Community Health Center, which also serves as the clinical site. This provides the students with real life experience in caring for patients and offers patients a low cost option for preventive dental care.
