Do you need health insurance?

We can help.

Our goal is to help you navigate and obtain the best and most cost-effective health insurance. If you are uninsured, we can help you apply for the health insurance that makes the most sense for you and your family.

Many individuals and families can get help paying for health coverage. Our trained and certified staff is here to answer your questions and help you apply for:

  • MassHealth
  • The Health Safety Net
  • ConnectorCare and other health insurance through the Massachusetts Health Connector
  • MassHealth Buy-In Application
  • SACA – Senior Application (65 and older)
  • Sliding Fee Discount Program

For us to best assist you and your family, especially during the COVID-19 pandemic, it is important to call first before coming into the Health Center. We can provide screenings and information over the phone, which can help you prepare for your first visit. It is helpful to have information about you and the people in your household who are applying for health coverage.

Once you have an appointment, please bring:

  • Photo ID (driver’s license, passport etc.)
  • Proof of income (two weekly pay stubs or one bi-weekly pay stub, income tax forms)
  • Proof of Immigration Status (legal permanent resident card, visa, asylum, etc.)
  • Proof of Citizenship (a photo ID along with a birth certificate or a passport)

*You may qualify for health coverage regardless of your immigration status. We can help you apply even if you do not have this information.

If you DO NOT have health insurance right now:

  1. Contact our Health Insurance Enrollment Team to set up an appointment.
  2. Bring: proof of identity, proof of residency, all sources of income.
  3. Based on your income, you will be enrolled in a health plan that you can afford.

We don’t select plans for you. We guide you through the selection process so you can select the health insurance that is the best fit for you and your family.

At Community Health Connections, no one will be denied access to services due to inability to pay.

CHC uses a discounted/sliding fee schedule based on family size and income. There are also many insurance options available to help you pay for you and your family’s healthcare needs. We can help you determine what you are eligible for and help you complete the application process.
