Community Health Connections (CHC) is a multi-site, non-profit community health care center offering primary family medical care, pediatrics, walk in urgent care, preventative and restorative dental care, oral surgery, behavioral and mental health services for children and adults, psychiatry, substance use disorder treatment, optometry eye care, optical shops, adult & pediatric podiatry, TMS & Spravato Clinics, a Children's Behavioral Health Center (designed just for children) and Outpatient Day Treatment. CHC is mission-driven, providing compassionate, high quality health care regardless of income or health insurance status. CHC serves all – insured, underinsured and uninsured, low income, publicly housed and homeless populations.
As a community health center, we provide the convenience of multiple services under one roof, at each of our community health centers. Our team-based approach allows providers to collaborate, making it easier for patients to receive coordinated, comprehensive care that supports their physical, mental and emotional health - at each of our community health centers, located in the cities of Leominster, Fitchburg & Gardner, Massachusetts.
