Leominster Community Health & Urgent Care Center offers the following services; Primary Care/Family Medicine, Medical Urgent Care, Pediatrics, Dental, Adult & Child Behavioral and Mental Health, Psychiatry, Intensive Outpatient Day Treatment, Optometry & Optical Shop (walk-in, open to the public), Podiatry, Nutritional Counseling, Patient Services/Health Insurance Assistance, Substance Use Disorder Treatment, Lab and Spravato Clinic.

Entrance A: Urgent Care, Medical, Pediatrics, Podiatry, Nutrition, Lab and Health Insurance Enrollment Services/Patient Services. - handicap accessible.

Entrance B: Behavioral Health, Dental, Optometry & Optical Shop, Spravato Clinic & Behavioral Health Day Treatment Program. - handicap accessible
