Community Health Connections can help you along your journey. As a Federally Qualified Health Center (FQHC), we provide the convenience of multiple services under one roof, at each of our community health centers. CHC offers the following treatment and support services at each of our health center locations:
Outpatient Based Addiction Treatment (OBAT)
Medication For Opioid Use Disorder (MOUD)
Medication-Assisted Treatment (MAT) for alcohol use disorder, substance use disorders
Contingency management
Individual or group counseling
Case management and care coordination and support
Weekly Adult Substance Use & Addiction Support Groups are free, you do need to be a CHC patient to attend. All are welcome!
