Primary Care and Family Medical care is the foundation of your overall health and wellness. At Community Health Connections, our primary care providers offer comprehensive care for patients of all ages, from routine check-ups and preventative screenings, to the management of chronic conditions. We partner with you to create a personalized health plan focused on keeping you healthy, at every stage of life.

Our provider care teams provide a full range of services including annual physicals, immunizations, sick visits, women's health, pediatric care and ongoing treatment for conditions such as diabetes, high blood pressure and asthma. 

At Community Health Connections your provider team takes the time to listen to concerns, answer questions and help you make informed decisions about your health.

As a community health center, we offer the convenience of many service lines, under one roof. When appropriate, our primary care teams can coordinate your care with behavioral health, dental, pharmacy and speciality services such as podiatry, optomety and nutrition, to ensure your care is connected and seemless.

This team based approach makes it easier for you to receive the right care at the right time- supporting your physical, mental and emotional well being in one trusted location.

Does CHC Offer Medical Programs That Serve Specific Populations?

CHC provides medical services to everyone who walks through our doors. CHC also offers programs and services that support the care of special populations, including:

  • Student Health Services for Fitchburg State University (FSU) Students – CHC partners with FSU to provide health care services to all FSU students. Click here for more information.
  • Healthcare for the Homeless and Public Housed program – CHC has comprehensive and integrated medical, dental, behavioral health, and substance use treatment services to those who are homeless and in unstable housing. Click here for more information.

Do you offer medical services offsite?

CHC offers virtal care - telehealth visits for some medical services.

How Do I Stay In Touch With My Provider?

You can sign on to our MyChart Patient Portal to connect with your provider, ask non-urgent medical questions, check lab results, request prescription renewals, and access your medical record, make or cancel appointments and so much more! CHC now offers Virtual Care -Telehealth visits. (Telehealth services may vary depending on your insurance carrier.)

*CHC shares the Epic/MyChart Electronic Medical Record with the UMass Memorial Health Care (UMMHC) system in order to facilitate inpatient and outpatient care. CHC is not affiliated with UMMHC and your communications are with your CHC provider team only.
