Primary care addresses your overall health, from your head to your toes, and all the worries in between. Our care teams work with you to create a personalized health plan so you can live your healthiest life.

Our medical services include:

  • Preventive care including health assessments and screenings
  • Physical exams for school, work, and sports
  • Chronic disease management
  • Pediatric care including well child checks and immunizations
  • Treatment of acute and urgent issues for children and adults
  • Women's Reproductive Health Services

Does CHC Offer Medical Programs That Serve Specific Populations?

CHC provides medical services to everyone who walks through our doors. CHC also offers programs and services that support the care of special populations, including:

  • Student Health Services for Fitchburg State University (FSU) Students – CHC partners with FSU to provide health care services to all FSU students. Click here for more information.
  • Healthcare for the Homeless and Public Housed program – CHC has comprehensive and integrated medical, dental, behavioral health, and substance use treatment services to those who are homeless and in unstable housing. Click here for more information.

What payment options do you accept?

At Community Health Connections, no one will be denied access to services due to inability to pay.

CHC accepts a variety of insurances and payment options, which may help you pay for expenses not covered by your insurance. If you are underinsured or uninsured, we can help you apply for the health insurance that makes the most sense for you and your family.

CHC also offers a Discounted Sliding Fee Program, which is based on family size and income. Please call our Patient Services team at 978-878-8100,  and choose option #3.

Do you offer medical services offsite?

CHC offers virtal care - telehealth visits for some medical services.

To see if you can connect with your/a care team through telehealth, please call 978-878-8100 and ask if your provider offers telehealth visits when making your appointment.

How Do I Stay In Touch With My Provider?

You can sign on to our MyChart Patient Portal to connect with your provider, ask non-urgent medical questions, check lab results, request prescription renewals, and access your medical record, make or cancel appointments and so much more! CHC now offers Virtual Care -Telehealth visits. (Telehealth services may vary depending on your insurance carrier.)

*CHC shares the Epic/MyChart Electronic Medical Record with the UMass Memorial Health Care (UMMHC) system in order to facilitate inpatient and outpatient care. CHC is not affiliated with UMMHC and your communications are with your CHC provider team only.
