Dr. Daryl Mc Clendon is a board certified Family Medicine physician, he completed his undergraduate studies at the University of North Texas and earned his medical degree from Western University of Health Sciences, College of Osteopathic Medicine of the Pacific, and completed his residency training at St. John’s Episcopal Hospital in New York City, New York. Dr. Mc Clendon has specialized training in Family Medicine, Neuromusculoskeletal Medicine, and Urgent Care, allowing him to provide comprehensive and practical care in a wide range of clinical settings. His professional interests include adult medicine, women’s health, pediatrics, telemedicine, and urgent care medicine. Growing up in areas with limited access to healthcare profoundly shaped Dr. Mc Clendon’s path into medicine. Witnessing the real world consequences of delayed or unavailable care inspired him to become the kind of physician patients could rely on, knowledgeable, prepared, and present when they need answers most. This philosophy continues to guide his patient centered approach to care. He currently resides in Worcester, MA. Outside of medicine, Dr. Mc Clendon enjoys traveling, writing music and poetry, singing, photography, cooking, camping, hiking, and biking. Shea went on to complete advanced training through a Doctor of Medical Science program at the University of Lynchburg and also holds an MBA in Healthcare Management from MCPHS University.

Kelley Prendergast, PA-C, is a board certified Physician Assistant dedicated to providing compassionate, patient centered care with a focus on longterm health and wellness. She completed her undergraduate studies at Western New England College and earned her graduate degree from the Massachusetts College of Pharmacy and Health Sciences. Born and raised in the Greater Gardner area, Kelley has a deep connection to the local community and a lifelong interest in medicine. She chose a career in healthcare to help people improve their quality of life and believes strongly in prevention, education, and treating the whole person, not just symptoms. Kelley maintains active membership with the NCCPA, AAPA and the Massachusetts Association of Physician Assistants (MAPA). She also serves on the board of Making Opportunity Count, supporting initiatives that strengthen community health and opportunity. 

Outside of work, Kelley lives in Westminster, with her spouse and their three children—including twins—as well as their dog and cat. Isadora Eller, FNP-C, is a Family Nurse Practitioner with a strong commitment to compassionate, patient-centered care. She earned her undergraduate education at Worcester State University and the Massachusetts College of Pharmacy and Health Sciences, and completed her graduate studies at the University of Massachusetts. She also completed a Family Nurse Practitioner residency with Community Health Connections. Isadora now leads the CHC Nurse Practitioner Residency Program.

Isadora’s training includes a bachelor’s degree in biology with a minor in chemistry, a bachelor’s degree in nursing, and a master’s degree as a Family Nurse Practitioner, providing her with a strong scientific and clinical foundation. She is passionate about preventive care and believes that consistent, evidence-based habits - such as staying active, taking medications as prescribed, and attending follow up visits, play a critical role in long-term health.

She earned her undergraduate degree from Fitchburg State University and completed her graduate education at Rivier University, followed by a residency at CHC. Heather’s passion for medicine was inspired early on by her grandmother, who was a nurse, as well as by caring for a chronically ill father. Originally born in Michigan and raised in Connecticut, Heather now lives in Winchendon, with her husband and their three children. Outside of work, she enjoys hiking in the White Mountains, gardening, and spending time outdoors with her family. Linet Migiro, FNP, is a dedicated Family Nurse Practitioner committed to helping patients improve their health through compassionate, comprehensive care. She earned her undergraduate degree from Salem State University and completed her graduate nursing education at Simmons University. Linet completed her residency at Community Health Connections. Her clinical interests include the management of diabetes, hypertension, hyperlipidemia, and mental health.

Born and raised in Kenya, Linet moved to the United States in the late 2000s, where she continued her nursing career. She now lives in Westford, with her husband and their two sons. Outside of work, Linet enjoys gardening and sharing home-grown fruits & vegetables with her neighbors, hiking in the White Mountains and local forest trails, and spending winter days sledding and tubing with her children.
