Shea Goudreau, DMSc, MBA, PA-C, is a Physician Assistant with clinical interests in family medicine and pediatrics. Shea completed his undergraduate studies at the University of New England and earned a graduate degree from the Massachusetts College of Pharmacy and Health Sciences University (MCPHS). Shea went on to complete advanced training through a Doctor of Medical Science program at the University of Lynchburg and also holds an MBA in Healthcare Management from MCPHS University.

Shea is actively involved in the profession, serving on the Board of Directors for the Massachusetts Association of Physician Associates and as an Adjunct Faculty Member at MCPHS University, contributing to the education and mentorship of future healthcare providers.

Kelley Prendergast, PA-C, is a board certified Physician Assistant dedicated to providing compassionate, patient centered care with a focus on longterm health and wellness. She completed her undergraduate studies at Western New England College and earned her graduate degree from the Massachusetts College of Pharmacy and Health Sciences. Born and raised in the Greater Gardner area, Kelley has a deep connection to the local community and a lifelong interest in medicine. She chose a career in healthcare to help people improve their quality of life and believes strongly in prevention, education, and treating the whole person, not just symptoms. Kelley maintains active membership with the NCCPA, AAPA and the Massachusetts Association of Physician Assistants (MAPA). She also serves on the board of Making Opportunity Count, supporting initiatives that strengthen community health and opportunity. 

Outside of work, Kelley lives in Westminster, with her spouse and their three children—including twins—as well as their dog and cat. In her free time, she enjoys boating, traveling, and spending time with family. Casie Short, PA-C, is a Physician Assistant with a special interest in nutrition and obesity management. She earned her undergraduate education at the University of Massachusetts Amherst and continued her studies at the Massachusetts College of Pharmacy and Health Sciences in Boston. Inspired by a desire to help people live healthy lifestyles, Casie encourages practical wellness habits, including taking a daily walk. Originally from Gardner, Massachusetts, she now lives in Lancaster with her husband and children. Outside of work, Casie enjoys reading, traveling with her family, and playing and coaching field hockey. She also serves as president of Nashoba Youth Field Hockey.

Isadora Eller, FNP-C, is a Family Nurse Practitioner with a strong commitment to compassionate, patient-centered care. She earned her undergraduate education at Worcester State University and the Massachusetts College of Pharmacy and Health Sciences, and completed her graduate studies at the University of Massachusetts. She also completed a Family Nurse Practitioner residency with Community Health Connections. Isadora now leads the CHC Nurse Practitioner Residency Program.

Isadora’s training includes a bachelor’s degree in biology with a minor in chemistry, a bachelor’s degree in nursing, and a master’s degree as a Family Nurse Practitioner, providing her with a strong scientific and clinical foundation. She is passionate about preventive care and believes that consistent, evidence-based habits - such as staying active, taking medications as prescribed, and attending follow up visits, play a critical role in long-term health.

Originally from Brazil, Isadora moved to the United States now lives in Worcester. Outside of work, she enjoys traveling and spending time with her family. Her motivation for pursuing medicine comes from a desire to support patients during their most vulnerable moments and to be a trusted presence during times of uncertainty.

Heather Aho, NP, is a dedicated Nurse Practitioner with a strong focus on family medicine and whole person care. She earned her undergraduate degree from Fitchburg State University and completed her graduate education at Rivier University, followed by a residency at CHC. Heather’s passion for medicine was inspired early on by her grandmother, who was a nurse, as well as by caring for a chronically ill father. Witnessing firsthand the impact compassionate healthcare can have on individuals and families motivated her to pursue a career where she could make a meaningful difference in patients’ lives. She has a particular interest in helping patients improve both their physical and mental well-being, with a strong belief in the role of exercise and lifestyle choices in preventing and managing a wide range of health conditions.

Originally born in Michigan and raised in Connecticut, Heather now lives in Winchendon, with her husband and their three children. Outside of work, she enjoys hiking in the White Mountains, gardening, and spending time outdoors with her family. A hands-on problem solver, she also enjoys tackling home improvement projects-from building decks to kitchen renovations. Heather is a member of the American Association of Nurse Practitioners (AANP) and volunteers at her children’s school as a teacher helper, reflecting her strong commitment to both family and community. Heather encourages her patients to stay active, noting the benefits of movement for everything from depression and arthritis to overall long-term health.

Kate Kish, FNP, is a compassionate Family Nurse Practitioner dedicated to providing comprehensive, patient centered care. She completed both her undergraduate and graduate nursing education at Regis College, with additional undergraduate coursework at the University of Massachusetts Amherst. Kate further expanded her clinical experience through a residency with Community Health Connections.

Kate’s clinical interests include nutrition, pediatrics, and women’s health, with specialized experience in Nexplanon insertions. She is passionate about helping patients feel heard, supported, and empowered to take an active role in their health. Her approach to care emphasizes prevention, education, and partnership, with the goal of helping patients become the healthiest versions of themselves.

Kate grew up in Massachusetts and currently lives in Littleton, with her husband, their two young sons, and their big, fluffy dog. Outside of work, she enjoys hiking, photography, and family trips to the zoo. A fun fact patients often enjoy learning about her: she can say the alphabet backwards—fast.

Faith Farnham, NP, is a Nurse Practitioner with clinical interests in family medicine and pediatrics. She completed both her undergraduate and graduate education at the University of New Hampshire and has specialized training focused on providing comprehensive primary care.

Faith brings prior experience from hospital based settings, including surgical floors and the emergency department, which shaped her strong commitment to preventive care and improving access to primary healthcare. She is passionate about helping patients avoid preventable hospitalizations and building meaningful, long term relationships to support their health goals.

She lives in Westford with her fiancé and their cat, Loki. Outside of work, Faith enjoys reading, crafting, cooking and baking, and spending time outdoors hiking, paddle boarding, and relaxing at the beach. She is also a certified Zumba instructor.

Alyssah Gentry, FNP-BC, is a Family Nurse Practitioner with a clinical focus on weight management, diabetes, women’s health and substance use disorders. Alyssah attended Anna Maria College and Massachusetts College of Pharmacy and Health Sciences. After six years working in emergency trauma care, she joined CHC and enrolled in the CHC Family Nurse Residency Program, graduating in 2024. Alyssah has a passion for prevention and helping patients make sustainable changes that support long-term health. Alyssah is an advocate for movement and encourages her patients to start where they are, do what they can, and recognize that small changes can make a meaningful difference. Alyssah also co-leads our Weight Management & Diabetes Education Patient Groups at CHC. A first-generation college graduate whose family immigrated from Laos, she brings dedication, resilience, and a personal perspective to her work. Alyssah is a competitive powerlifter who enjoys strength training, staying active with her three dogs, and spending time with her husband and daughter. Kelly Goncalves, PA-C, is a Physician Assistant with a professional interest in college health. She earned her undergraduate degree from Fitchburg State University and completed her physician assistant studies at MCPHS. A resident of Fitchburg, Massachusetts, Kelly is a member of the American Academy of Physician Associates (AAPA) and the Massachusetts Association of Physician Assistants (MAPA). She is also active in her community and a United Way volunteer. In her free time, Kelly enjoys reading and working on jigsaw puzzles.

Alexandria Harding, FNP, is a Family Nurse Practitioner with a clinical interest in diabetes care and a strong commitment to patient advocacy. She earned her education from Kaplan University and Purdue University Global and completed the Community Health Connection FNP Residency Program. Linet Migiro, FNP, is a dedicated Family Nurse Practitioner committed to helping patients improve their health through compassionate, comprehensive care. She earned her undergraduate degree from Salem State University and completed her graduate nursing education at Simmons University. Linet completed her residency at Community Health Connections. Her clinical interests include the management of diabetes, hypertension, hyperlipidemia, and mental health.

Born and raised in Kenya, Linet moved to the United States in the late 2000s, where she continued her nursing career. She now lives in Westford, with her husband and their two sons. Outside of work, Linet enjoys gardening and sharing home-grown fruits & vegetables with her neighbors, hiking in the White Mountains and local forest trails, and spending winter days sledding and tubing with her children. A fun fact her patients enjoy: she is the head chef in her household. Linet is actively involved in her community and volunteers with the Pathfinder’s Club at MVSDA Church. She is also a member of the American Nurses Association (ANA) and the American Nurses Credentialing Center (ANCC).

Dr. Daryl Mc Clendon is a board certified Family Medicine physician, he completed his undergraduate studies at the University of North Texas and earned his medical degree from Western University of Health Sciences, College of Osteopathic Medicine of the Pacific, and completed his residency training at St. John’s Episcopal Hospital in New York City, New York. Dr. Mc Clendon has specialized training in Family Medicine, Neuromusculoskeletal Medicine, and Urgent Care, allowing him to provide comprehensive and practical care in a wide range of clinical settings. His professional interests include adult medicine, women’s health, pediatrics, telemedicine, and urgent care medicine. Growing up in areas with limited access to healthcare profoundly shaped Dr. Mc Clendon’s path into medicine. Witnessing the real world consequences of delayed or unavailable care inspired him to become the kind of physician patients could rely on, knowledgeable, prepared, and present when they need answers most. This philosophy continues to guide his patient centered approach to care. He currently resides in Worcester, MA. Outside of medicine, Dr. Mc Clendon enjoys traveling, writing music and poetry, singing, photography, cooking, camping, hiking, and biking.
