Patients are able to make pediatric appointments by scheduling an appointment through our Medical Department.
To become a patient you will need to fill out New Patient Forms.
You can either fill out those forms online or or you can stop at any of our locations and request a new patient packet and make an appointment. You may also contact our Patient Call Center at 978-878-8100.

CHC is now offering June Well-Child Clinics! (In addition to our August & September Clinics)
The Well Child Clinics are Walk-in/No Appointments Needed/First-come, First-served and will be happening at our community health centers in Leominster, Fitchburg & South Gardner.
These clinics are for CHC patients & those who do not have a primary care provider and need to establish care.
