As of September 12, 2022, CHC’s Medical and Behavioral Health Services will be moved from 14 Manning Avenue to our new site at 165 Mill Street, Leominster, MA 01453. | Dental Services will still be located at 14 Manning Avenue in Leominster, MA.
CHC began using Epic as its Electronic Health Record and MyChart as its patient portal on July 25, 2022. Stay tuned for more information as this transition continues.
Starting Friday, August 5, 2022, the CHC COVID-19 Vaccine Clinic at 326 Nichols Road in Fitchburg will be closed on the first Friday of every month.
CHC is now using Care Everywhere, a tool that allows the exchange of Healthcare information. Read our Notice of Privacy Practices to learn more about how Care Everywhere can help you and your family.
All CHC locations will be closed on Thursday, November 24, 2022 and Friday, November 25, 2022. CHC will reopen for regular business hours on Monday, November 28, 2022.

Due to the decrease in testing volume, CHC's COVID-19 Testing Clinic at 375 Nichols Road will be closed as of Friday 2/25/22. CHC will no longer be offering public testing at this time. CHC patients will still have access to COVID-19 testing as needed.
COVID testing in Gardner is reserved for CHC Patients only and will occur at our main Gardner facility at 175 Connors Street.

Community Health Connections works with various schools and institutions of higher learning to offer clinical experience within the unique care environment that our community health center provides.
The post-graduate Family Nurse Practitioner (FNP) residency at Community Health Connections offers the opportunity for professional development and additional skills training in a community health center.

Each year New York University’s Langone Health System Dental Residency Program sends dentists to Fitchburg Community Health Center to further their training. These dentists use their skills helping to care for patients while learning about the daily operations of a busy dental practice.
