Everyone is welcome as a patient at CHC because everyone deserves quality care and to live their healthiest life. We want to help and are ready to provide you the services you need! 

What Should I Expect As A New Patient? 

Quality care. As soon as you walk through our doors.  

How Do I Become A New Patient? 

To become a patient you will need to fill out New Patient Forms.

You can either fill out those forms online or or you can stop at any of our locations and request a new patient packet and make an appointment.

You may also contact our Patient Call Center at 978-878-8100.

Directly below, you will find New Patient Forms to click on and fill out online.

These forms are available in English, Spanish and Portuguese. 

Once you have successfully filled out the forms and submitted online, you will receive an email confirmation that your forms have been submitted to CHC from AssureSign. (Please check your junk/spam folders for confirmation email.) 

You will receive a phone call within 5 business days or sooner from CHC to set-up your first appointment.

If you don't have insurance, the Patient Services team can be reached by calling the main CHC number 978-878-8100 and choosing option 6 for health insurance enrollment.

How Do I Stay In Touch With My Provider?

You can sign on to our MyChart Patient Portal to connect with your provider and access your medical record.

MyChart* helps you conveniently manage care for yourself and your family members by connecting you to your medical, behavioral health, and optometry care at Community Health Connections whenever you need it.** 

With MyChart, you can see lab or test results, ask non-urgent medical questions to your provider, send photos and documents, schedule or cancel appointments, and more!
