Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral and mental health services for children and adults, substance use disorder treatment, specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, high quality health care regardless of income or health insurance status. CHC has five locations within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts.

To become a patient you will need to fill out New Patient Forms.
You can either fill out those forms online or you can stop at any of our locations and request a new patient packet and make an appointment.

These forms are available in English, Spanish and Portuguese.
Once you have successfully filled out the forms and submitted online, you will receive an email confirmation that your forms have been submitted to CHC from AssureSign. (Please check your junk/spam folders for confirmation email.)
You will receive a phone call within 5 business days or sooner from CHC to set-up your first appointment.

At Community Health Connections, no one will be denied access to services due to inability to pay.
CHC accepts a variety of insurances and payment options, which may help you pay for expenses not covered by your insurance. If you are underinsured or uninsured, we can help you apply for the health insurance that makes the most sense for you and your family.
CHC also offers a discounted/sliding fee schedule based on family size and income.
