Community Health Connections partners with Fitchburg State University to provide healthcare services to all students. Through this partnership, Fitchburg State University students have access to all the services offered by CHC at any of its locations.

While students are welcome to see any of our providers at any of our five sites, Kelly Goncalves, PA-C is a provider dedicated to seeing Fitchburg State University students at our ACTION location at 130 Water Street in Fitchburg which is less than a mile from campus. Medical services at the ACTION site are provided from 8 a.m. to 5 p.m. on Mondays, Wednesdays, Thursdays, and Fridays, 8 a.m. to 8 p.m. on Tuesdays.

On the weekends, students can access services at our Leominster Community Health & Urgent Care Center open on Saturday & Sunday, from 9:00am-5:000pm. (Leominster Community Health & Urgent Care Center is located at 165 Mill Street, and is on the MART bus route. Bus #3.)

Will The Services I Receive At CHC Be Covered?

The cost for accessing care will usually be determined by what type of insurance you have.

  • For students covered by MassHealth, most of the services will be provided at no cost (students will be required to designate Community Health Connections as their primary care provider); · For students enrolled in the FSU’s Blue Cross and Blue Shield student health insurance plan, the office visit copayment is $15; or
  • Students under a parent's plan should be aware that Community Health Connections participates with most insurance plans and the cost for medical visits and ancillary services will be determined by the insurance plan’s schedule of benefits.
