Community Health Connections is all about community.

We work best when working together with community organizations, supporting each other for outreach, education, care, and support for just about every need an individual could have. Our partners range in specialty and size, yet are all incredible at what they do and how they serve the community.

Connecticut River Valley Farmworker Health Program

Community Health Connections partners with the Connecticuty River Valley Farmworker Health Program to offer health services to migratory and seasonal agricultural workers within our service area. Learn more about our Migrant Health Program.

Growing Places

Community Health Connections partners with Growing Places to have CHC’s Nichols Road location serve as a weekly stop to purchase fresh produce. Everyone is welcome to shop anytime.  

Winter 2023 Farmers Markets are the second Thursday of the month from 11:45AM-1:00PM. January 12th, February 9th, March 9th,  April 13th and May 11th. Questions? Contact Growing Places 978-598-3723.

Fitchburg Art Museum

Community Health Connections partners with the Fitchburg Art Museum to showcase local art from the museum at our Fitchburg, Gardner, and Leominster locations.

Fitchburg State University

Community Health Connections partners with Fitchburg State University to provide healthcare services to all students.

Groton Hill Music Center

Community Health Connections partnered with Groton Hill Music Center to offer live music sessions twice a month at our Fitchburg Nichols Road location throughout 2021.
