MyChart* helps you conveniently manage care for yourself and your family members by connecting you to your medical, behavioral health, and optometry care at Community Health Connections whenever you need it.** 

With MyChart, you can see lab or test results, ask non-urgent medical questions to your provider, send photos and documents, schedule or cancel appointments, and more!

*CHC shares the Epic/MyChart Electronic Medical Record with the UMass Memorial Health Care (UMMHC) system in order to facilitate inpatient and outpatient care. CHC is not affiliated with UMMHC and your communications are with your CHC provider team only.

With MyChart, you will have the ability to:

  • See Lab and Test Results.
  • Ask Non-urgent Questions to your Provider Care Team. You can send a picture or document, too.
  • Schedule, View or Cancel an Appointment with your PCP or a Specialist.
  • Easily Add your Appointment to your Calendar. All it takes is three clicks to add your connected calendar.
  • Speed Up the Check-in Process. Use the precheck-in feature to answer questions and setup your co-pay.
  • Request Prescription Renewals from your Provider's Office. When no more refills at your pharmacy.
  • Upload, Request and Receive Forms. Return to work, camp physicals, immunizations and more via myChart.
  • Update your Information. Make changes to your address, phone, email, insurance, emergency contact person.

How to get MyChart

  • STEP 1: Give your email address and ask for an activation email to be sent to you. Anyone at your provider’s office
    or central scheduling at 978-878-8100 can send this to you.
  • STEP 2: From the activation email create your myChart account by entering:
    • at least a 6-character username (case insensitive)
    • at least an 8-character password (upper and lower case with numbers and letters, case sensitive)
    • your date of birth
  • STEP 3: Get the benefits of direct access to your clinical information.

Use the MyChart Mobile App

  1. Download the myChart mobile application from the Google Play Store or Apple App Store, enter myChart in the search field. Select the icon and then click on Get button.
  2. Find and select the Community Health Connections logo from the list of organizations.
  3. We recommend setting up a TouchID or Passcode for using myChart mobile.
