Community Health Connections operates 5 community health centers, 2 are located in Fitchburg, MA. Fitchburg Community Health Center, 326 Nichols Road (Old Burbank Hospital Campus) and ACTION Community Health & Urgent Care Center, 130 Water Street.

Services provided at ACTION Community Health Center; Primary Care/Family Medicine, Medical Urgent Care, Dental, Adult & Child Behavioral and Mental Health, Substance Use Disorder Treatment, Fitchburg State University Student Access to Health Care, Homeless & Publically Housed Services, Patient Services/Health Insurance Assistance and Lab.

Primary Care has acute appointments available daily at each site that can be scheduled. To schedule for a same day appointment, call 978-878-8100.

Urgent Care services are available here, including Leominster Community Health & Urgent Care Center and South Gardner Community Health & Urgent Care Center.

Using the secure patient portal MyChart App, you can now save a space in line at any CHC Urgent Care location.

Three convenient options to become a new patient;  fill-out a online new patient form , call us or stop at any of our health centers and request a new patient packet. Our front desk staff can help you and set-up your first appointment.

On March 5, 2024 CHC opened a dedicated space for Children's Behavioral Health at 130 Water Street. The new center offers services to children and adolescents, ages 5-20 years old.
