When you're a Community Health Connections (CHC) patient, you're part of a community.

As a community health center, we often talk about “comprehensive care.” It's a phrase you hear frequently in healthcare, but here at Community Health Connections, it truly defines how we care for our patients. Every single day.

Our model as a Community Health Center is built on three essential pillars; Medical, Dental & Mental Health. Each one matters on its own, but the real impact happens when they come together under one roof.

In our medical offices, behavioral health isn't a separate service down the road, or a phone number handed to a patient at the end of a visit. It's integrated into care at every one of our health centers.

At CHC, we have an integrated behavioral health clinician, Justine Paoletti, LMHC, LADC that works side-by-side with our medical providers, right in the same space. When a medical provider recognizes that a patient may benefit from additional support, we don't just make a referral; we make a connection. It's a warm handoff, a personal introduction. Care begins in that moment, not weeks later. That's what comprehensive care looks like.

During a routine dental visit, a patient may discuss stress or anxiety. During a medical checkup, a provider may notice signs that something deeper may be going on. These moments matter, and because we have behavioral health services embedded within our health centers, we can respond quickly, thoughtfully, and with compassion. This is especially meaningful as we recognize Mental Health Awareness Month. It's a reminder that mental health is health.

As a physician and Chief Medical Officer at CHC, this is what gives me confidence in the comprehensive care we provide and that our patients are not navigating a fragmented system.
