I’m Susan Lowe, Director of Patient Services. Our mission is simple but powerful, to care for everyone in our community regardless of their health insurance status or income.

We are here to help with health insurance enrollment assistance

Navigating health insurance can be overwhelming whether you're applying for the first time or renewing your coverage. Our team at CHC offers free personalized enrollment assistance which will help you understand your options through the Massachusetts Health Connector. (MassHealth/Medicaid/CHIP/SACA/HSN)

Myself and my staff will walk you through every step from determining eligibility to submitting your application, ensuring you feel confident and supported during this confidential process.

Sliding Fee Discount Program

We all understand that for many families, cost is one of the biggest barriers to care and that's why at CHC we offer a sliding fee discount program, designed to make our services affordable for everyone. This program is based on income and family size. Our Sliding Fee Discount Program helps reduce the cost of medical, dental and behavioral health services. Even if you have insurance, you may be eligible, and all patients are encouraged to apply.

Everyone needing assistance will be scheduled an appointment with a Certified Application Counselors. Applications counselors will review the required information over the phone with any required documentation you may need to provide . Appointments are available at each of our health centers in Leominster, Gardner and Fitchburg. All of our health centers are located on the MART bus routes.
