I'm Ellyn Guinding, Chief Nursing Officer at Community Health Connections and I oversee our Substance Use Disorder Treatment Programs, which includes outpatient services and connection to recovery-based services for those struggling with addiction.

One of the grants driven to help people in their recovery is the RIZE Massachusetts grant.
Thanks to a grant from RIZE Massachusetts, we have expanded our services to provide additional support to individuals who have survived an overdose. Through the Linkages to Care (LinC) RIZE grant, we are now able to send our staff into local hospitals and treatment centers to meet with overdose survivors and set-up the care they need. Our goal is to prevent a second overdose and connect patients with the care they need to ensure lasting recovery. (FACT: Over 60% of overdose patients will relapse.)
This initiative strengthens our commitment to meeting people where they are—whether that's in the hospital, at our health centers, or in more challenging environments like on the streets or under bridges.
Our efforts to bridge the gap between hospitals, outpatient services, and community-based organizations are crucial for ensuring that individuals who survive an overdose continue to receive the care and support they need to avoid relapse.
Annie Belmer, a Recovery Coach is a team member who works tirelessly to support patients and to ensure they receive the care necessary for their recovery. Her unwavering commitment highlights the compassion and support at the heart of our work.

At CHC, we offer a range of services, including:
  • Outpatient-Based Addiction Treatment (OBAT) for flexible care.
  • Medication for Opioid Use Disorder (MOUD) to manage opioid dependence.
  • Medication-Assisted Treatment (MAT) combining medication and counseling for substance use disorders.
  • Contingency Management to reinforce positive behaviors.
  • Individual and Group Counseling to offer emotional support and coping strategies.
  • Case Management and Care Coordination to help individuals navigate healthcare services.
  • Gym access - recovery focused
  • Recovery coach support
  • Peer lead recovery groups
  • Harm reduction and overdose prevention
These services are available at all CHC locations to ensure accessible, comprehensive care.
