My name is Alexis DiGeronimo, PharmD and I am the Clinical Pharmacist, and Certified Diabetes Care and Education Specialist at Community Health Connections. November is National Diabetes Awareness Month, an opportunity to learn what diabetes is, who may be at risk, and how to have meaningful conversations with your health care team about prevention and treatment.

As a clinical pharmacist and diabetes educator, I am committed to helping every patient feel confident and supported in managing their health, whether they have diabetes or are working to prevent it.
Prediabetes is more common than many people realize, and it’s an important early sign from the body. This stage provides a valuable opportunity to make lifestyle modifications that can impact the your health. With the right support, many people are able to prevent or delay the development of diabetes through these thoughtful food choices and incorporating movement into their daily routine.
For patients living with diabetes, my role is to provide education, guidance, and medication support tailored to each person’s needs. I work closely with the primary care providers to assist them in treating and educating patients. From self monitoring blood sugar to continuous glucose monitors (CGMs), helping them understand their readings, set achievable goals, and feel more in control of their day-to-day health.
My goal is to help each person build the skills and confidence needed to manage diabetes safely and effectively. I’m here to help make diabetes care easier to understand, more manageable, and something you don’t have to navigate on your own.
Since medication support goes hand-in-hand with nutrition and lifestyle, we offer monthly Diabetes Group sessions at our Leominster Community Health & Urgent Care Center. At these group sessions, we review strategies for managing diabetes, seasonal nutrition discussions, open the floor to any questions that patients may have, and offer CGMs at no cost for attendees.
I know that managing diabetes can feel overwhelming at times, especially when get busy. My role is to provide guidance, encouragement, and clear information so CHC patients feel supported as they work toward their health goals.
