On February 25, 2002, CHC opened its doors as a primary care practice in Fitchburg, providing medical services to the underserved. Now, twenty years later, CHC has four sites across Fitchburg, Gardner and Leominster, offering a variety of services and ready to care for anyone walking through our doors!
